As an established provider of services for people with disability, Ability Centre understands and supports the importance of a lifetime approach to providing individualised services and supports.

Ability Centre's Support Coordination service supports NDIS participants and their families to make the most of the services you can access through your funding from the NDIS.

Taking the necessary time to connect and be guided through your NDIS experience with you support coordinator.

This aims to strengthen your ability to connect with informal, mainstream and funded supports and to increase your capacity to maintain support relationships, resolve service delivery issues and participate in NDIA processes.

Coordination of Supports

Gaining support that targets your immediate needs to find solutions and to give you a range of options, whether it's collecting potential provider information or linking you in with community groups near you.

This aims to strengthen your abilities to connect to and coordinate informal, mainstream and funded  supports in a complex environment. This includes resolving points of crisis, developing capacity and resilience in your network and coordinating supports from a range of sources.

Specialist Support Coordination

This is an intensive and reactive service using specialised coordination in times of possible or potential crisis.

This aims to focus on addressing barriers and reducing complexity, while assisting you to connect with supports and build capacity and resilience.
